but if you put your cursor over the purple heart medal in the stats page

it says requirement is to have a 1:4 kill/death ratio with a minimum of 5 kills and 20 deaths

shouldn't it be the other way around?

No, purple heart is for wounded in combat - it means you suck at combat

No, it's very common for someone to get a purple heart in bf2; it's because you just like die too much and dont get as much kills. In real life, it's usually very highly regarded, but in bf2, you really don't want to get it. Some people who don't have it should keep it that way. But then again, you want to get all the badges, ribbons, and medals so yeah, w/e.
