You need to achieve the following badges;

Veteran Armor Badge, Veteran Transport Badge, Veteran Helicopter Badge, Veteran Aviator Badge, Veteran Air Defense Badge, Veteran Ground Defense Badge.

Once you achieve these badges you'll get promoted.

Only thing I can think of is his account was hacked to 4 stars and it got reset but he kept his rank, so really he'd be a colonel, like the account you posted in the op.
