yea, at a restaurant I usually just double whatever the tax is and add just a little if the service is good. Tax is 7% here so it works out between 15-20%.

I'll tip at a restaurant if the service is good.
I'll usually tip a cabbie to the nearest couple dollars, unless he was annoying (drives to slow, talks too much shit, tries to go the wrong way).
I'll sometimes chuck in some change into the tip jar when drinking at a bar, if they have one and if the service is good.
Like I once said to someone when I worked as a valet in a hotel and they asked if they were "supposed" to tip me: it's not expected, but it is appreciated.

I must say I've never really had god-awful service before, so this is all conjecture.
I did once. And the food was terrible, though previously it had been fine. We walked out with a close to $200 bill outstanding. The waitress chased after us with the manager, we stood our ground and he took over the situation after we argued with her. He apologised and offered a discount the next time we came back and said he'd waive the bill. We didn't go back, however.
