How come a shot to the head with a m16 (or weapon with similiar ammo) doesn't result in death while a knifestab to the ankle does?

Because the knife is 1-hit-1 kill to make it easier to get knife kills, and there's a damage multiplier for the head (3 or 4) and some weapons don't have enough damage to make a headshot a 1-hit-kill, even with the multiplier.

And you might have lagged and the game wouldn't have registed it as a head shot

The knife was made a one-hit one kill weapon because of the close range necessity. If you made any bullet to the head kill, well, no-one would use a knife and it'd be all pistols. The game would lose flair (in my opinion).
Hope that explains something. Oh, and look at http://wiki.bf2s.com/weapons for how many bullets each weapon takes to kill an enemy (a headshot = 3 bullets).
