What does it mean if you have received an award, but it does not show up, and it is underlined??
Does that mean you did earn it, but the server failed to register the award??

No, the line under it (assuming you mean the bf2s stats page) just indicates that you need to fulfull the IAR requirements to get the award.

i.e. 10 kills IAR as Anti Tank to get Basic AT badge.

The award in question is the Combat Infantry Medal. They are all checked off, and their is no IAR stat.
So does that mean the server is a piece of &%**&^????

It means you haven't earned it yet. You have to do the requirments to get it.

And it doesn't matter what booster pack/expansion pack you get it in.

Assuming this is you: http://bf2s.com/player/45284040/ then I would assume next time you log in to play, you will get it as you have only just clicked over the 200 hours mark. If you start playing and don't have the time standards up yet, then you need to get over the amount required, then log out and log back in again.

does it count if you complete the IAR requirement, but not the total time requirement? i.e. i've killed 40 people iar as an engineer, but not fulfilled the time needed for the badge... do i have to complete the iar requirement again?

yes. global requirements must be fulfilled either in the same round as the IAR ones, or before the IAR ones.

You have to complete all global requirements, i.e. time, total heals, total resupplies, etc., before you can try and complete the IAR requirements. Like for me, I can get expert medic and assault if I get 40 kills in a round with each kit.

Almost, the global reqs have to be filled before u start a new round. U could start a round with 99 minutes as a kit and not get the award that round. U would have to wait till the next round. Your global stats don't update till the end of the round. (sometimes later) Even if u leave mid game, your stats won't update till the server u were on ends the round.
