In viewing my stats I noticed that the Requirement for my Middle East Service ribbon have been met... but I have not received the award yet, could EA have messed up, because BFHQ does not show it either...

Also, why doe some awards have bars under them and others dont?

Its a bug, really. Those are the conditions (that the community has agreed is correct), yet it doesn't straight away work for some people. Just keep playing on those maps and you get it eventually. Some believe you need to play both armies on each map, etc.

As for the bars, they mean you've fulfilled the Global requirements, and all that remains is you fulfil the In-One-Round (IAR) requirement.

You must complete the Global requirement (and it be said on BF2s/BFHQ) before you can do the IAR.

It also could be that the server was unranked?
I had to win some twice.

yeah, it happens. Some people get it, others don't, some still get it without the met criteria...

It's the buggiest award in the game.

Yes it is a bug, some people get the ribbon, some dont.
