Without, but then me and my GF have been together for almost 5 years now. And there is a huge difference with or without, no matter what they say.
My girlfriend and I use the pill and a condom. We've done it without a condom many times, but only finished with a condom a few times. Neither of us is ready for a kid at all, so that's why we double the protection.

Random one-nighter - Definitely with. There's absolutely no point in risking getting STD:s and/or the partner pregnant. Quite frankly, it's not only stupid, but I'm guessing that most of you wouldn't have the balls of a) go in for testing nor b) call the other party involved later, that you have aquired an STD.

Usually people learn after they got something (and gotten rid of it), but then there are the dumbasses who just keep on pushing their luck and end up being spreaders of one or several diseases, only because of their own carelessness.
Now, in a monogamous relationship, and if the other involved party is a) on the pill or b) you are trying for a baby, then without. It just is that much better.
Never believe the one you're about to have sex with, if they claim that they've "been tested, and cleared", and never use your own negative results as an argument to go without a rubber. It's just careless.
