As mostly when a lot of something is required, you need long lasting Titan rounds, which means nearly equal and active or inactive teams. You should start up by either attacking the enemy titan guns, destroying assets or trying to be quicker silo capping than the rest of your team. Every time you die, spawn on the titan to check if the guns have been destroyed or at least attacked. Start repairing, but be aware: the score limit of 16 (I think) points is active since 1.0.5, so don't waste your time repairing the guns after you brought 8 of them back from "destroyed" to "operational".
Once the titan shields are down, it's a matter of observation. Does the enemy team attack a lot or are they all camping in their titan? Grab your Recon kit with APMs and RDX and try for a little while to see how many points you make that way. Remember, every kill equals 1 point for the kill and one teamwork point for titan attack or defend bonus no.
It's not going on fast enough because the other team is shitting their pants and only camping without moving anywhere, therefore not triggering APMs or running into one of your RDX traps? Go back on the ground. Cap silos till you feel all dizzy from running in circles. Most likely, if the other team does not only consist of braindead boy scouts, there'll be at least a few guys trying to go for the silos, too, so you can be chasing each other from silo to silo.
Here's the screenshot of me getting mine, you'll see that the round was pretty much over already and I think I actually had already 57 or 58 teamwork points, so you better make sure you have some time buffer...
