You need to have played the commander position for 50 hours total
You need to (after both those are met) go into a server and command, get 45 command points in that same round, and voila

I never teamswitch, but my w/l is pretty good. I do teamswitch on maps where the opposing team will obviously lose though, but thats at the start of the map.
