I think it's an interesting idea to see which that 5 stars album is for you, a perfect one, a must to have.  And I also think it would be interesting to discover those bands and albums that aren't in the charts, but are also jewels, such as underground bands or side projects too.  So, if you'd like, say which the perfect album is, and which is that album maybe unknown for most of us, that deserves to be known as well.

In my case my perfect album is Dirt (1992) from Alice in Chains.  It's a real masterpiece.  If you never listened to it please take an hour for 13 incredible songs, about heroin addiction and death.  The two voices of Layne Staley and Jerry Cantrell combines perfectly to create IMO the best album I ever listened.
The unknown jewel, perhaps a lot of you know it, is Temple of the Dog (1990) from Temple of the Dog, a one album project in tribute to late singer of Mother Love Bone, with Chris Cornell, Stone Gossard and Jeff Ament, and the contribution in some songs of Eddie Vedder and Mike McCready.

I'm only 16 but listen to lots of music and i've never come across a 5 star album. Basically because i have never found an album where i like all the songs. I'm a very open listener and have atleast a few albums + randoms songs from each major music category. Probably the closest thing so far for when would be the Eminen Show. I enjoyed everysong on there and i still go back to them.

I'm not quite sure how many people know it but I'd say they are not that well known to the majority.

my 5star musthave is Leitmotif by Dredg.

It's their first album (aside from some EPs n stuff) and it's simply brilliant imo. The tracks a very experimental at some points which also makes it very interesting. "The story of Leitmotif is about a man with a "spiritual disease" who must seek out different cultures of the world to find a cure for his affliction." (copied the description).
The album was released in 1999 and it's about 54 minutes long.

Other than that I can also recommend De-Loused in the Comatorium by The Mars Volta. Very experimental sounds too but I like that

Dirt. Agreed. or AIC unplugged show. Perfect performance

Another few well known bands would be:

TOOL- Lateralus
Nine Inch Nails-Pretty Hate Machine
System of a Down- Toxicity
Pink Floyd- The Wall
A Perfect Circle- 13th Step
Soundgarden-Badmotorfinger
Stone Temple Pilots-Core

Some of those are pretty obvious, so some less popular would be:

One Minute Silence- Buy Now...Saved Later
Static X- Wisconsin Death Trip
Ministry- In Case you didn't feel like showing up (live)
Primus- Sailing the Seas of Cheese
Widespread Panic- 'till the medicine takes [hippie-ish band NOT heavy metal]

hmm, alot of my posts about music are the same really.  I don't get turned onto new music very quickly.

It is difficult to find that perfect album, but rock operas are always a great option. The Who invented them, and Pink Floyd has been famous for their use. That is a perfect way to make an album, so that they are a connected piece of work put together excellently. Still, other albums have been great without the necessary storyline or continuous stream.

Personally, The Who's Who's Next is my favourite, an absolute classic.
