Usually because my first instinct is to kill the enemies when I see them. Only after that first instinct do I take note of the weapon in hand, and only then do I consider a different option.
However, that just doesn't happen very often...

Depends on circumstances.
If FF is on, try carefully to shoot the enemy.
If it's off, indiscriminate blasting.
If I'm a medic, blast away and heal/revive my teammate.
Assault, noobtube.
AT, shotgun the buggers.
Otherwise, nade them. Unless I'm testing out a new weapon, in which case I'll try and get a bead to get a hang of the aiming.
