Longest word in an actual dictionnary is "Pneumonoultramicroscopicsilicovolcanoconiosis" which basically means a severe lung disease caused by over-inhalation of volcanic smoke and/or ash.

Biggest one ever ranks in at one hundred eighty-nine thousand eight hundred nineteen (189,819) letters. It's a protein. The shortened version is called Titin. Wont post the long one cause it kinda exceeds the character limit.
