Ok.
recently, I have not been able to go to sleep before 12 no matter how much I try.
When I sleep, I wake up frequently, and I am always aware of my surroundings (I dream, but I can wake up whenever, and I know I will wake up.)
Also, I cannot get real sleep until 4:00a.m., and when I wake up, I feel like shit.
When I am at school, I tend to doze off frequently.
Do I have any sleep disorders?

Go to the store and buy some melatonin. Try it. You can't overdose on it, but it helps you fall asleep (but not stay asleep).

If that doesn't work, go see the doctor and request a sleep study.

Also, I am very fit, and I exercise daily.
I will try out the medicines.

Exercise 5 or 6 says out of the week and go to bed at 9 or 10. If that doesn't work, then try the meds.

OK, i have many sleep disorders including apnoea, night terrors, etc

Make sure ur tired before u go to bed.
if u go to bed and not sleeping within 15 mins get up.
dont play computer games brefore bed.
make sure ur room is dark and quiet and not hot.
dont read or watch tv in bed.
no caffeine after 6 or switch to decaf totally
go to a sleep and respiratory specialist (get tested for apnoea)
They might also prescribe some sleeping tablets.
Dont take stilnox!!!

you should not watch TV or play games an hour before you go to sleep.
try to take 2 pauses of 20 minutes whilst doing nothing during the day ( this way your mind will be trained to rest faster)
Also when you sleep don't put your mobile phone to close to you.
and drink more water.

Your body regulates its sleeping patterns with melatonin. That's why people take it to help them sleep. What happens is your body produces melatonin naturally when you're not exposed to light, especially your eyes. It stops the melatonin production when you are exposed to light (ie. daytime). To help reset your internal clock, start turning down the lights a couple of hours before you want to sleep. Then open the blinds, turn on the lights, or spend some time outside as soon as possible after waking up. Skip the dark sunglasses if it's not too bright.

simple to fix... nyquil  death green flavor  one dose and your out cold for 8-10 hrs.

best way ive found to reset my internal clock is to stay up 24 hrs get so tired and you crash you feel great when you wake up and the next day you go to sleep at the same time.  say your normal wake up time is 6 am.
skip a nites sleep and the next nite by 10 pm your out cold. by 6 am next  day your up and going and your internal clock is reset.. well at least for me it works. 
me i sleep 5-6 hrs a nite and im good.  make sure you sleep area is dark no light at all. sound what ever helps you sleep me i use a box fan let it run on high and blowing  on me and zzzzzzzzzzzz

its not about the length of sleep but quality if you get 8-10 hrs of restless sleep over say 6-8 hrs of deep  sleep you feel like crap over being good to go all day.
you learn the true meaning of life when you see houses and cars blow by like leaves on a windy day.

My sleep patterns are even more screwy than that - I occasionally get like 8-12hrs sleep per 'day', but more often than not, it'll be 12-16hrs per 'day' , where each 'day' involves between 8 and 16hrs awake...

And I get insomnia all the time.
