Get that person to give you a list of his/her achievements and that should be a good starting point. From there just write it like it's an informative letter occasionally complementing the person in question
Try:
To whom it may concern,
In all the years I've known [insert name here] he has never once been on time for anything. He is lazy and passes responsibility onto others and avoids work at all costs. He enjoys office politics and turning employees against each other, often successfully cutting productivity. If you hire him ensure that you never let him meet your wife unless you enjoy a polygamous relationship. He is one of my best friends and great supporters.
