And besides all of that a doctor sets out with the goal of saving a patient's life, sometimes he can't do that or possibly doesn't do his job properly resulting in the death of the patient, but at the end of the day the death was never intended.
A torturer sets out with the goal of inflicting physical pain and harm on a detainee, too much physical pain and harm can result in death and yet the torturer does his task regardless of this risk. Death in these cases only result when the torturer has dished out too much physical harm and as such the death is very much on purpose - i.e. the result of a deliberate action and not an accidental one.
