When i ahd mine done they did 2 small incisions and then pulled it out of a bigger one near belly button

He was on some painkillers for a tooth, and that masked the symptoms so they had to do exploratory surgery to find out if that was the problem. If you know what it is in advance, you can just make a few small incisions and pull it out.
