did they talk with chinese accent and did the MEC talk with the middle eastern accent, or did they have the same voice over as the U.S. guys in the game

in Options there must be a option : English Voice Only or something like that

Yeah in options there is an option for English V.O. Only.

After hearing the other MEC/PLA clips for so long, you just kinda know what they mean.

I dislike the English VO Only because it makes it harder to distinguish between what your teammates and your enemies are saying, like if I hear the cry "MEDIC!" I know that's a Marine and not my fellow communist soldier.  Otherwise, I'd have to check my mini map to see if there's a friendly in the area to revive or whatnot.

I've played like this for ages but as mentioned by deject there are disadvantages. For example, if you're capping a flag prone behind cover and you hear "Frag out!" how do you know it's not an enemy throwing it at you?

It'll take a while to learn all the common phrases in the other languages so you don't have to look at the left of the screen but it might be worth it. If you're playing as MEC or Chinese and you hear English from around a corner - even just "Ammo, here!" or "Get first aid, here!" - then you'll know there's at least one enemy nearby; same thing in reverse if you hear any Chinese or Arabic.
