Yes, he did kill his wife. The entire time throughout the movie he was enabled to live out his fantasy world because the head doctor or warden or whatever let him so that he might snap out of his delusion. In the end he conciously understands what he did, and he hates himself for it. So when his personal doctor tests him at the very end of the movie, he acts like he regressed into his old fantasy world, when in reality he hasn't, but he can't live with the fact that he killed his wife, and he would prefer a frontal lobe lobotomy.
