Saw it this weekend. Read for some more explanation than above^^

The whole "I'm a US marshal thing" is not real, the guy used to be a US marshal but went crazy (lives in a fantasy world) and is now a patient on Shutter Island. He is actually Laedis, the same guy he is looking for on shutter island (the one who supposedly set his apartment on fire, killing his wife). In his dreams he keeps seeing his wife and a young girl, this girl keep telling him he 'should have saved her'. The girl is his daughter. The patient who vanished from shutter island, the woman who drowned her three kids is imaginary and represents his wife. His wife was the first one to go crazy, she says she has a bug in her head. The apparent fire was actually real, but the wife started it, and she did survive. They then moved to a lake house, and the wife drowned the three kids they had. After this the Marshal shot his wife, created his fantasy world and was sent to shutter island. The whole movie goes around one big Role playing game, every person the marshal comes into contact with is part of this role playing game. Some of them have even been created by the marshal self. Through this genius role playing game (thought of by the doctors ofcourse) the marshal comes to realise he lives in a fantasy world. The doctor then tells him this isn't the first time he was made to realise this, but that he regressed back into his fantasy world and became violent every time. He then warns him that if this happens again, a lobotomy will be preformed. The marshal says he understand that he is actually Laedis, and that his wife killed their kids, making him go nuts. The movie ends on a scene of his personal doctor (this guy plays the marshals partner in the fantasy world) asking the marshal some simple questions, the marshal has indeed fallen back into his fantasy world and is taken away....for lobotomy I guess

He did not actually fall back into his fantasy world at the end.  He just goes along with it because he knows his personal doctor is testing him to see if he is crazy.  He decided that he couldn't live with himself because he killed his wife and didn't save his children, so he makes it seem like he has fallen back into the fantasy world.  However, his very last line is something like "Is it better to live as a monster or die as (forget the rest)" which shows that he realizes what is going on.

edit: well...I might have seen it just for Leo...

The ending is the best part, but even though you know what happens I would still say go and see it, it was a good movie

except that she's working. she dam sure would, except to go from Florida to Michigan in early spring is a tough sell.
