Penis enlargement (can't believe I'm taking this seriously) nearly never works.
Those pills you see advertised in magazines like "Men's Health" and whatnot, first of all, have not been approved by the FDA or any other government or medical health body (in other words, they could be just sugar pills). Even if they did what they claimed to do (which is increase blood flow to the corpus cavernosa, the hollow tubes in the penis that are filled with blood during an erection), this wouldn't make the penis LARGER, unless you're so hopeless that you never get a fully hard erection in the first place. The best the pills can do (if indeed they do work) is give you a harder (NOT longer or thicker, maybe by about 1/4 inch or so at most) hard on.
Pumps work on the same principle. The pressure differential causes blood to flow to the penis, thus causing engorgement. Unlike pills they actually can work, however they have a few catches:
1) Typically they won't make much of a difference--about 15% is typical. For the average 5.5" erection that means you'll gain an entire three-quarters of an inch! Yay! </sarcastic>
2) For the vacuum to work properly you must shave your pubic hair. This causes itchiness and sometimes ingrown hair pimples.
3) There is a very real risk of injury involving something called "penile fracture"--I think you can figure out what that means. It doesn't sound very pleasant.
4) Most damaging: The gains are temporary, meaning you'll have to apply the pump EVERY TIME you want that extra 3/4 of an inch. That's real sexy. And you thought putting a CONDOM on was a turn-off...
