So I just installed it, played it for a couple of minutes just to see if it works. It asked me to do hardcore mode, so I did. Does it add any more realism to it?

hardcore has been very very manageable for me but i'm a careful, slow playthrough player

then chug water and eat food whenever you've fast traveled/rested/waited
you have to babysit your companions a little in combat too

I have about 20k caps at the moment and it's all I need.
I never need to spent anything repairing because I have the perk that lets you repair things using 'similar' things. So I can repair my armour with pretty much anything, and same goes for my rare guns.
