korea and taiwan are like that too. afaik they both routinely occupy the no.1-2 spot in world healthcare rankings. walk into basically any clinic without an appointment and you can get the whole works for a couple hundred quid.

haven’t interacted with the japanese health system yet, but i imagine it’s the same also.

had a full bloodworks, urinanalysis, physical scans and exam while being prudent this winter.

really low on vitamin b12, but otherwise all good. kind of a miracle considering how i'm still basically staying out all hours and putting whatever the hell i please into my long-suffering frame.

hopefully 3 months of pills will sort out my low-lying fatigue.

totally free in the UK, and for a tiny notional (<$10) fee in south korea, taiwan, and japan ... (for basic diagnostics like a blood test and urine analysis i mean, not CT scans or MRI or whatever).

pretty regular for east asians to check themselves into hospital overnight to top up their fluid levels and rest. admittedly that's because they're suicidally overworked and sick leave/pay is still stuck in a 1950s level of cultural acceptance; but still.

being proactive about healthcare and making the basics accessible saves a shitload of money for reactive care and emergency interventions later down the line. who wudda thunk it?!

there's actually a funny area of tokyo that was built to a kind of 'suburban ohio' template, which makes it look ridiculously out of place in high-density, higgledy-piggledy tokyo with its characteristic aesthetic.

there's an 'america-town' in osaka too, which is kind of like a hipster area full of craft beer joints and vintage clothing stores. actually one of the better neighbourhoods in the whole city, imo. very cool vibe there.

i'd say about 80% of the US servicemen i've encountered out here – by far most of them are in korea, or at least that's where they're most visible and allowed to roam off base the most – were literally like 19 year old kids. visibly young to look at. probably their first time ever abroad. extremely hard to be intimidated by them, and kind of puts all the drunken drama and trouble they get into in a different perspective. they're the hayseed flyover state versions of coastal elite greek fraternity undergraduates.

i've heard horrendous stories about military marriages and whatnot. no thx.

That reminds me that I was showing my kids All Quiet on the Western Front (1979) today. We got to the scene where the bald teacher hypes the kids to go to war. I paused it and told the kids "it sounds like the same stuff I tell you about the military and China, right? The more things change the more they stay the same." A few kids were watching the movie and may have internalized the message.

I too have a BB12 deficiency, not sure if its just diet or some medical issue yet.

Mind blown that I'm dependent on some weird chemical which has cobalt at its core.

ironically my liver health, blood sugar, cholesterol, etc. are all in within very good ranges.

ditto BMI and body fat.

i probably do objectively drink far more than your average 36yo at this point – more circumstantial than anything else; i'm living in tokyo and going out to music venues a few times a week whereas half my friends have moved out to the boonies to their wives and incoming children – but look younger and have a healthier liver than most of them. truly a genetic lottery. great metabolism and genes (god bless those 13th century knights errant).

i do practically zero exercise, and my diastolic blood pressure is a little higher than average for my age (systolic within normal, healthy range). probably should take up a brisk walk every day to break out of my near-permanent state of WFH inactivity (i'm very much seasonal and fairweather about these things at the moment, hiking and mountainclimbing during the months of good weather but very much a slug when its too humid/cold).

the lack of B12 is a mystery to me, but i'm not a vegan and consume meat several times a week (lack of dairy/meat produce being the standard etiology). but all the symptoms are a perfect match with my general description w/r/t frequent fatigue, advanced grouchiness when tired, low levels of energy and motivation, etc. its been kind of hard to disentangle those symptoms historically from the near-permanent process of recovering from the latest hangover or comedown. so that means likely it's some sort of absorption or gastrointestinal thing. i did zap myself with heavy courses of antibiotics a few times in my 30s for one thing and another, and it did take me a while to build back up a good gut biome ... (thanks korean kimchi and japanese yakult).

wouldn't surprise me tbh if frequent cocaine use ate up half the good vitamins and minerals in my body in my 20s. and lord knows what else it was being cut with. probably stripped my marrow like a rare-earth mine. i've read that B12 levels can take 2–5 years to build up and be spent; it's a very long term thing, so not something you necessarily notice you're deficient in overnight, or with local/recent changes to diet or lifestyle.

think i’m at about 160 mmol, with 200-950 or something being the normal range.

tbh if depleting a very cheaply renewable resource is the only cost of hard partying well into middle-age, i’ll take it. my heart, liver, vital functions, extremely high IQ, affable charm and evident charisma, etc., all seem intact. i’m not seeing any moral in the story here.
