Its just one of those "when you notice it you keep noticing it" things, i bet nearly all of us have some pixels out of place.. we just don't tend to notice them/look for them.

I guess, it's the only real logical explination. And that black dot everytime I die must be the focus point for whenever anyone dies.

If you truely are worried about your card, run a program like 3dMark or similar and let your machine render images for several hours. Check it every so often and if you start getting artifacts, return the card and try a brand like Gigabyte. (I run dual Gigabyte 6600GTs, and have never had any problems).
I get the same problem every now and then with my 6800 with graphics on high, so it can not be much to worry about. I have a few dead pixels in my monitor but after hours of playing I do not notice it much anymore. Stop worrying about it and you will stop noticing it.
