Sounds right, although I'm a bit hazy on the details.

If you kill a player ten times during a single run where you yourself don't die, and that's more than he's been killed similarly by someone else, then you become his worst enemy. I think that's right.

But the system is buggy and this is reflected in the lists on the various stats pages; there's definitely some flaw in the way it gets recorded, as has been discussed here a couple of times.

I doubt it would be just total kills of a given player during a round. Otherwise we'd all have massive lists I'm sure.
