Well hiding more carefully, if possible, is a good start. But you have to protect your own ass when commanding since nobody else is going to do it. Probably every player in the world who has any significant amount of command time has been killed while hiding (although it's a lesson you learn pretty quick!)
For any map where the commander is vulnerable (Karkand is one of the worst, for the commander on either side) he needs to run scans constantly and pay attention. On the game I got vet ground defence in, on Karkand, I protected my ass, and my assets, about five times from a spec ops player.

I don't think that having the map semi-transparent would be a good idea overall since it can be hard enough to see in detail what's going on at times.

I agree in general that commanders should command, but it does depend on the map and the player load. My highest score was achieved as fighting commander on Warlord, IO, with 14 players and the 54 kills I made (many of them flag defences) sure as hell helped my team to win.
